Palomar Heights Post Acute Rehab took such amazing care of my mom-in-law, Heather, during her battle with end-stage Alzheimer’s/Dementia. They were so compassionate to us during our many visits and communicated with us well when we were not able to be there. From Heather’s case managers, social workers, and the many nurses and staff, such as Anita, Isabel, Debbie, Nakia, Erica, Jessica, and so many others. They made sure she was safe, even moved her to another room to make sure she had the best roommate, Coralee. We thank you from the bottom of our hearts! You are truly angels on earth!

I have been complaining for 3 months about the lack of communication between my adult disabled son in this hell hole. Angelo the DON said my son is refusing my calls. That’s not possible because they refuse to charge his phone and let him have access to it. Yesterday, I found an Ativan (schedule 5 drug) on his floor. The med nurse had no idea when he last had it, and no one would tell me. The asst DON tried telling me that when they try to get my son to call me he asks if he has to talk to me. This is absolute B.S. He demonstrated yesterday that he has the ability to use his phone. For 3 months, the dietician wasnt aware he could feed himself. Seriously? A few men who are residents there almost got into a fight. They are so short staffed. The neglect is horrible. The higher ups do nothing but grin at you and look like English is their third language. This us nothing more than modern day gulag.

My disabled son has been here for 3 months. I have proof of the severe neglect and isolation forced upon him. They refuse to charge his phone, they refused to get him out of bed for 2 months. I had to provide him with a chair. I live an hour and a half away and have called the police to do a welfare check on him three times. I have call logs where I have called the facility dozens of times, but they don’t answer the phone. It took 2 days to do STAT x rays for my son after he complained of a stomach ache. I left his social security paperwork with the case manager, but she quit. They left it sit on her desk for 2 months before giving it back to me, unfilled by the doctor. I have pleaded to speak to my son’s doctor for 3 months, but never hear a thing. The call bells are ignored. The food sucks. This place is a hell hole. I am trying to move my son elsewhere, but I get nothing but excuses and lies. Avoid this place like the plague. The higher ups are only interested in profit and protecting the facility. They do not care about human life

From the highly skilled clinical professionals – including registered nurses, certified nursing assistants, and rehabilitation nursing assistants – who expertly manage complex medical needs, to the compassionate physical therapists, diligent kitchen personnel, welcoming receptionists, and the essential maintenance and environmental services teams, each individual contributes significantly to an exceptional care environment. Their collective professionalism, tireless work ethic, and genuine care are, in my experience, unmatched. While the challenging and dynamic nature of acute and long-term care can occasionally present demanding situations, what consistently shines through is their steadfast resolve and empathetic approach. The staff unfailingly responds to every concern with responsibility, compassion, and thorough follow-through, ensuring that any issues pertaining to my son’s well-being and comfort are addressed meticulously. My prior experience in navigating various long-term care facilities for my mother-in-law – an endeavor that required several transitions before finding a suitable option in San Diego – provides me with a clear benchmark for excellence. Indeed, should I ever require specialized long-term care myself, Palomar Heights Acute Nursing and Rehab would be my unequivocal and immediate choice. It is my heartfelt conviction, forged through years of direct observation and deep gratitude, that no other facility could provide a higher level of compassionate and professional care for your loved one.

For your family sake, please do not have them stay here for care. Very understaffed. Noone answers the phone. Very disorganized and you will not get a Doctor to communicate with you as they only visit the patients once/twice a month. Food is slop tasting and it looks like it. Grooming care is poor for the patients. Found my nephews toothbrush on the floor and dirty gloves as well. Unexplained scratches on his legs that noone could explain why. If I could I would leave no stars.

My best friend was a resident at this facility and my friend wanted to give me his health care directive and power of attorney and we had the meeting at their facility but after the meeting Elisabeth Duran (soceal services assistant) said that my best friend was evaluated upon arrival and was found to be incapacited and my authority as his agent or representative was not valid! She also would not allow my friends brother to visit him because he complained about the food. I explained my friend was selling the trust that held his late father house to pay for his funeral, now he is dead and I cannot bury him with the amount of money i have but would have been remedied had we sold the house. Her boss Mason Varney would not return my emails stating these facts. If they would have just given my friends brother the evaluation for the close of escrow it would have been fine but instead they made unsubstantiated claims and now my friend is in a refrigerator and we are waiting for the death certificate. My god have mercy on your souls.

My son is in this awful facility. For 16 days, his agonizing pain was completely ignored. His floor was covered in sticky goo. There is hair all over the floor. His call bell was stuck in the bed frame for at least 2 days. I mentioned it, but nothing was done until I informed them this is illegal. My son is just left laying in bed. His pain is ignored. He is 6’3″ and they allow him to be slumped down in the bed. He was given a nicotine patch because he said he smokes a pack of cigarettes a day. He also says he is trying to stop a train from running away, and thinks he is in different countries. It took a week of demands to have that patch removed. I was told various lies regarding this patch. They tried to put the blame on the hospital he was at before being put in this facility. I cannot get a straight answer from anyone. I just get stupid looks and lies. My son has been out of bed once in 17 days. His glasses are constantly out of his reach. The neglect is atrocious. Nobody cares because he is on medi-cal. I called the ombudsman and we will be having a meeting with the staff. I would avoid this place like the plague if you have a choice.

I can’t say enough about the care and compassion the team at Palomar Heights showed my mother during her stay. From day one, it was clear that patient care here isn’t just a priority — it’s part of the culture. Kalany (morning shift) and Daniel (afternoon shift) both stood out for their authenticity, professionalism, and genuine passion for caring for others. Their energy reflects a standard that’s clearly shared across the entire staff. Every interaction I witnessed reflected kindness, consistency, and a deep respect for patients. As I begin my undergraduate studies in Business Management (Organizations track) at Cal State San Marcos, I couldn’t help but notice how rare it is to see such a strong, positive culture consistently carried out at every level of an organization. Most importantly, every time I left, I had complete peace of mind knowing my mom was in good hands — and that means everything. Thank you to the entire team at Palomar Heights. Your work truly matters.

not a great experience signing in that soured my visit but the place is very well maintained

I would not wish this place on my worst enemy. Grandmother was sent there for rehab, and aside from the 15 minutes of PT and 15 minutes of OT she was either kept in a chair or bed the entire time. Three people to a room, it smelled and honestly management could not get their act together. Worst part was, two hours after arriving they lost her hearing aid, and magically two and a half weeks later it showed up. They said it was found in her dresser drawer which had extensively been searched prior to this by myself and two others. DO NOT PUT YOUR LOVED ONES THERE.

The only reason why they are getting one star it’s because it’s not allowing me to put 1/2 or even 1/4 of a star which is what this place deserves for the way they treat our loved ones. All the staff in this facility (Nurses and administrators) don’t care about how our family feels ever. They never pay attention to our loved ones. My family had to take our family members out of this hell hole because he was ignored for hours while he was in excruciating pain. He was ignored for hours by incompetent, insensitive, heartless staff. I will never recommend this place to anyone unless you want your family to suffer.

Palomar Heights Post-Acute took such great care of my family member. The staff were kind, attentive, and always made us feel comfortable and supported. The building is clean and welcoming, and the therapy team really helped with recovery in a way that felt personal and encouraging. You can tell the people here genuinely care about what they do. I’d absolutely recommend Palomar Heights to anyone looking for quality post-acute care. The administrator Skyler was really really really really really really nice!

All employees are very helpful and positive.I wanted to coms add more info to my review. All the staff nurses, CNAS and OT/PT were all very helpful and caring. Samantha helped when I got my clothes she put my initials and room# on them. She was always pleasant and we had good times especially when she would sometimes get help from Elizabeth. I liked putting my 2 cents in when they were talking. I was there 4 months so you get attached to some people I miss so many. Marytess, Catalina, Alina, Kelly, Marsha, Desiree the med nurses. CNAS, Jessy, Gabriel, Jesica, Raquel, Kalany, Arlet, Bryan, Isabelle. Last but not least my Physical therapy family. Marcello, Melanie, Doug, Clarissa, Ruben, Amanda .I almost forgot Rudy. That’s what happens when you get old and also have brain surgery to correct your brain bleed cuz you fell out of bed broke your femur and hit your head.

I found the staff to be, not only friendly, but attentive and accommodating… even with the more high maintenance patients…. They were always, cheerful, patient and informative, while guiding you through the day. Making sure that you are getting the most out of your stay and the care that each individual needs… I am happy to be going home, but if I ever find myself needing special care again, I would want to come back and spend that time with all of you… So I just wanted to say : Thank you to all the staff, I appreciate you and would recommend you to all and anyone.

Staffing was excellent and they really care.

This facility really has, not only the care, but also the appeal of a 5 star building. The remodel looks great.
